Differential Diagnosis of Three Causes of Pathological Nocturnal Emission and Easily Confused Manifestations

Pathological nocturnal emission
Most sexologists currently believe that nocturnal emission is a normal physiological phenomenon and a normal part of human sexual behavior. There is no pathological nocturnal emission; in other words, nocturnal emission is not a disease.
Their investigation revealed that young men with higher levels of education and better economic conditions are more prone to nocturnal emission.
Single men have a higher rate of nocturnal emission than married men, and the incidence of nocturnal emission is significantly higher among incarcerated men than among free men.
Nocturnal emission itself does not have much impact on physical health.
However, if psychological changes such as anxiety occur as a result, you should go to a hospital's sex counseling clinic for examination, find the cause, and receive timely treatment.
Generally speaking, there are three possible reasons for this phenomenon:
(1) Lack of correct sexual knowledge, excessive focus on sexual issues, or frequent indulgence in pornography, such as being influenced by pornographic books and magazines, thinking too much about sex-related issues for a long time, being frequently in a state of sexual impulse, or having a bad habit of masturbation in the past, which causes the cerebral cortex to be in a state of sexual excitement, can lead to nocturnal emission.
(2) Certain diseases of the genitals can cause nocturnal emission due to adverse stimulation, such as phimosis and redundant foreskin, urethritis, prostatitis, seminal vesicle hyperplasia, etc.
(3) Physical weakness, overwork and other factors can cause dysfunction of various organs, especially the loss of control of the lower sexual centers by the cerebral cortex, while the excitability of the erection center and ejaculation center is increased, which is also a cause of nocturnal emission.

Symptoms easily confused with nocturnal emission
Nocturnal emission refers to ejaculation that occurs during sleep or while awake without sexual intercourse or masturbation.
Some people often mistake other conditions for nocturnal emission, so it is important to distinguish between them.
(1) During each penile erection, some people will often have mucus discharge from the urethral opening without ejaculation. The discharge will gradually decrease after the erection disappears. This is the result of the accessory glands secreting during sexual arousal. It is a physiological phenomenon and does not belong to nocturnal emission.
(2) Some people mistake the small amount of thick, clear secretions that leak from the urethra before ejaculation during intercourse and the semen that continues to be discharged from the urethra after intercourse for nocturnal emission. In fact, it is the secretion of the bulbourethral glands and the semen remaining in the urethra during sexual arousal. Don’t be nervous.
(3) When suffering from prostatitis, there is often dribbling after urination, and thin white fluid may flow out of the urethral opening, also known as "urethral dribbling". Do not mistake it for nocturnal emission.
Sometimes it needs to be differentiated from premature ejaculation and other conditions.

How does Traditional Chinese Medicine understand the etiology and pathogenesis of lower back pain during sexual intercourse?
Traditional Chinese medicine believes that lower back pain during sexual activity is mainly related to excessive sexual activity, external injuries, kidney essence deficiency, and stagnation of cold and dampness.
The etiology and pathogenesis are as follows:
(1) Kidney essence deficiency: Insufficient congenital endowment or prolonged illness leading to postnatal malnourishment can both lead to kidney essence deficiency and malnourishment of the waist and viscera.
(2) Stagnation of dampness: When the kidney's yang qi is insufficient, excessive sexual activity can cause cold and dampness to stagnate in the lower abdomen, blocking the meridians of the waist and causing pain.
(3) Sexual strain injury: Young people are vigorous and have excessive sexual activity, which can lead to waist injury, qi stagnation and blood stasis, and pain due to obstruction.

Traditional Chinese Medicine's understanding of the pathogenesis of penile pain during intercourse
Penile dysphagia is a condition characterized by pain in the penis during sexual intercourse.
The etiology and pathogenesis are as follows:
(1) When emotions are not in harmony, liver qi is stagnant, qi stagnation and blood stasis occur. When intercourse occurs at this time, the penile meridian is blocked, and pain occurs due to obstruction.
(2) If one is born with a weak constitution and is exposed to cold, the cold will congeal the liver meridian, obstruct the penile collaterals, and cause penile pain during intercourse.
(3) Excessive sexual activity or masturbation can damage the kidneys, leading to liver and kidney deficiency, malnourishment of the penis, and penile pain.

Etiology and pathogenesis of kidney wind
Kidney wind, also known as "scrotal wind," "kidney scrotum wind," "embroidered ball wind," "kidney wind," or "scrotal dampness and itching," is related to the liver, spleen, and kidneys. Pathologically, it involves the accumulation of wind, dampness, and heat pathogens in the scrotum. The specific etiology and pathogenesis are as follows:
(1) External pathogens: The three pathogens of wind, dampness and heat accumulate on the surface of the scrotum skin and do not dissipate, which is the cause of this disease.
(2) Disorders of the internal organs: When the functions of the internal organs are not in harmony, the functions of the liver, spleen and kidney are disordered, and damp heat evil flows down to the skin of the scrotum, which can also cause disease.
(3) Intolerance by nature and contact with external toxins: If a person is inherently intolerant to toxins, contact with paint, synthetic clothing, pollen, drugs and certain foods can cause this disease.

Traditional Chinese Medicine's understanding of azoospermia
Azoospermia is also known as "sexual syncope", "sexual azoospermia", "sexual wind", "horse riding wind", "horse dismounting wind", etc.
The etiology and pathogenesis are as follows: congenital deficiency, prolonged illness leading to deficiency of both liver and kidney, weak constitution, or working day and night to prepare for a wedding, or excessive mental exertion leading to physical fatigue, or being addicted to alcohol, which damages the heart and kidneys. If there is excessive excitement during sexual intercourse, the yin and yang will not be in harmony, resulting in illness.

Traditional Chinese Medicine's understanding of hematospermia
Hematospermia is also known in Traditional Chinese Medicine as "semen blood," "bleeding during intercourse," or "bleeding after each sexual encounter."
Its etiology and pathogenesis are as follows:
(1) Damp-heat in the lower burner: Excessive consumption of fatty, sweet, spicy and pungent foods, especially excessive drinking, leads to stagnation and damage to the spleen and stomach. As the spleen loses its ability to function properly, phlegm and dampness are generated internally, which can turn into heat over time. Damp-heat in the bladder can also cause stagnation and failure to resolve. Improper sexual intercourse can lead to the accumulation of damp-heat and fire toxins in the lower burner, which can disturb the seminal chamber, burn the blood vessels, and cause the essence and blood to descend together, resulting in this disease.
(2) Yin deficiency and fire excess: Damp-heat and fire toxins are not resolved, which consume yin fluids; excessive sexual desire and frequent masturbation deplete yin essence; yin deficiency generates internal heat, and fire excess forces the seminal chamber downward, damaging blood vessels and disturbing the seminal chamber, causing blood to leak with the seminal essence, resulting in this disease.
(3) External bleeding: Perineal injury damages the blood vessels of the seminal vesicle, causing internal obstruction of blood stasis; forceful intercourse forces the seminal vesicle to be released, damaging the blood vessels of the seminal vesicle, causing blood stasis and obstruction of the vessels, resulting in prolonged blood stasis and blood semen that does not heal over time. The disease progresses into the vessels and causes internal blood stasis.
Both can cause blood to flow out of its proper channels and overflow into the seminal vesicles, resulting in this disease.
(4) Deficiency of both spleen and kidney: Prolonged illness and physical weakness, as well as excessive sexual activity, can damage the spleen and kidney.
Spleen deficiency leads to failure to control blood, and kidney deficiency leads to failure to consolidate essence, resulting in the loss of both essence and blood, thus causing illness.
